Player Registration Form - 2007/2008 Season
Provo Blades Payment Policies
All players are responsible for paying their team fees online via paypal. This will allow you to pay either from your bank account, credit card, or through a paypal account balance.
Payments must be made on or before each due date or the player will not be permitted to play at the next practice or game.
Any payment that is made AFTER the due date will be charged a $5 late fee. You will receive an email invoice as a reminder if you have not paid by the due date.

Possible Additional Fees
Tournament fees will be an additional $50 each and will be posted with due dates as more details become available.
New players will need to purchase jerseys and socks which generally cost around $50. Once the jerseys have been ordered, the payment option will become available.
Returning players may be asked to pay a small amount for new socks. Details will be provided once they are available.
Team merchandise (t-shirts, sweatshirts, warm-ups, etc.) is sometimes available and is not included in team fees. If there is an order being placed, the payment option will be posted here.
